About the Role:
Reporting to Director of Clinical Services – Quality Assurance, the Quality Outcomes Coordinator is responsible for quality outcomes for the persons we support, and quality assurance measures to provide efficient and effective provision of supports and services for persons diagnosed with Autism Spectrum Disorder and their families. Responsibilities may include:
- Through actions, supports the Mission, Vision, Values and Strategic Plan of Kerry’s Place
- Develop and maintain positive relationships with families, partner agencies, volunteers, government employees and the community
- Provide support through evidence-based treatment models and approaches for persons diagnosed with Autism Spectrum Disorder
- Support the Clinical Manager with developing and coordinating quality related education
- Coordinate, monitor and maintain a caseload for Facilitated Person Directed Planning meetings with supported persons, support teams and families
- Monitor the work flow for planning and facilitation processes
- Trouble-shoot scheduling problems for Person Directed Planning process
- Electronically maintain data on the frequency of Person Directed Planning meetings
- Collect, analyze and evaluate information and programs pertaining to enhancement of quality life
- Coordinate the planning and implementation of organizational wide events to inspire, motivate and celebrate, such as the annual Outcomes Month activities
- Share positive outcomes with Communications department for newsletters and social media channels
- Ensure the health and safety of supported persons in accordance to Ontario Regulation 299/10 Quality Assurance Measures, the Child, Youth and Family Services Act, and the Provincial and Municipal regulations and by-laws
- Support the Management team to ensure professional and thorough records are maintained and up to date for all persons being served
- Responsible for liaising with Kerry’s Place Facilitator(s), Liaisons, Managers and support circle members as needed to ensure that processes run efficiently
- Measure and report on quality assessment tools, Person Directed Plans, and other quality and best practice initiatives
- Review the status of incident reports and provide feedback
- Assist with system improvement projects for the clinical department in coordination with the Clinical Manager
- Other duties as assigned
Education Qualifications:
- University degree or college diploma
Knowledge and Experience Requirements:
- 3 years of experience in human services
- Strong knowledge of the Developmental Services sector
- Knowledge of Autism Spectrum Disorder and evidence-based treatment models and approaches
- Completion of Person Directed Planning Facilitation training or equivalent is an asset
- Working knowledge of related legislation and regulations such as: Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act and Ontario Regulation 299/10, etc
- Competency and effective use of database systems
- French language is an asset
Working Conditions/Physical Requirements:
- Position requires flexible hours including evenings and weekends
- Must have the ability to complete the Non-Violent Crisis Intervention (NVCI) certification, First Aid/CPR as well as perform the required physical techniques. Training will be provided at no cost
- A valid Driver’s License, with a minimum G2 and access to a reliable vehicle is required
