Individualized, short-term support for Autistic teens 14-18 and their families experiencing a significant transition and living in the Peel Region.
Our six-session support program gives Autistic teens and their families the support and strategies they need to navigate unique transitions. Participants will use sessions to identify their goals and learn the tools to achieve those goals successfully.
Transitional scenarios appropriate for this support include, but are not limited to:
-Moving to a new home or school
-Returning to school after an extended absence
-Increasing attendance due to a school refusal
-Returning home from a hospital or other facility stay
-Increasing social skills
-Forming more daily living skills
-Needing community integration
-Seeking out housing and respite
What to Expect:
Families will first meet with an Autism Support Facilitator to discuss their needs and goals. During that first session, the Facilitator will conduct an assessment and connect families to an Autism Support Associate.
Families will meet with their Autism Support Associate for a maximum of 6 sessions to create transition strategies, set goals and work through those goals.
