Individualized, short-term support for Autistic teens, ages 14-18, and their families who are experiencing a significant transition and live in the Region of Peel.
This six hour support program gives Autistic teens and their families the support and strategies they need to navigate unique transitions. Participants will use sessions to identify their goals and learn the tools to achieve those goals successfully.
Transitional scenarios appropriate for this support include, but are not limited to:
- moving to a new home or school
- returning to school after an extended absence
- increasing school attendance
- returning home from a hospital or other facility stay
- developing social skills
- developing daily living skills
- community integration
- housing and respite
What to Expect:
Families will meet with an Autism Support Facilitator to discuss their needs and goals. During that first session, the Autism Support Facilitator will conduct an orientation interview and connect families to an Autism Support Associate. Following this meeting, families will meet weekly with an Autism Support Associate, for a maximum of six hours, to create transition strategies, learn new skills, and put supports in place for the identified goals.
