Your initial Clinical Information Meeting will help us recommend the best Individual Clinical Services to meet your needs. It is the first step to accessing Individual Clinical Services.
During the meeting, the clinician will gather information about your strengths, needs and goals so our team can create an individualized clinical recommendation to achieve your goals. The meeting is one-hour long and will be held virtually unless you request an in-person meeting at one of our resource centres.
When registering for your Clinical Information Meeting on My Community Hub, please select your preferred region for receiving the recommended Individual Clinical Services.
Clinical Information Meetings are only required for those that:
- have not already completed a Clinical Information Meeting in the past six months, or
- don’t already have a clinical recommendation on file from Kerry’s Place
